For Patients & Their Advocates
To qualify for participation in KCPA, patients must meet the following eligibility criteria.

Patients are typically between 18-64 years old. |

A patient’s household combined income must fall at or below 200% of the federal poverty guideline. |
|
• |
Patients must be referred by their primary care provider or a hospital. After the referral and enrollment form are received, the KCPA Case Manager will schedule the specialty appointment. |

• |
Patients who reside in King County will be seen by a King County provider. |
|
• |
Initial enrollment is for a six-month period. An extension may be granted, pending continued eligibility and treatment needs. |

How KCPA Works

Safety net clinics and private physicians provide primary care homes for patients. |
• |
Primary care physicians refer eligible patients to specialists and hospitals who, through KCPA, donate their services. |
• |
Physician volunteer commitments and patient referrals are managed through a KCPA Case Manager. |
• |
For tracking purposes only, services provided are recorded through standard insurance claim forms. |
• |
Patients receive their medications through their primary care providers or safety net clinics. |
